Registration Forms

Below are the intention forms for the 2025-2026 school year. All Alumni, Woods Church members, and New Families must complete this form and email it to register@woodscdc.org by the applicable dates listed on the forms to maintain their priority status. Your child must be the age of the class by September 1, 2025. Please use one form per child. Thank you.
